2010 Season Registration

  • Registration Form
    This main form for registering your player for the season includes an area to write any requests.

    Photo / Media Release and Shirt Size
    Pictures are taken during the games throughout the year. Parents must sign a consent form to have any photo of their child used on our Web site or other materials.

    Parent Code of Conduct
    All parents of participating HLL players must sign this important code of conduct

    Player's Code of Conduct
    All HLL players must sign the player's code of conduct

    Medical Release Form
    To be completed by parents of players informing the league of any medical issues, and containing important contact information in case of an emergency.

    Personal Health and Medical History
    Another required form for particpation in the league. It contains important medical history questions.

    Interested in becoming a sponsor of the league? With four sponsorship levels to choose from you're guaranteed a hit!
    Volunteer Application
    Anyone who wishes to participate as a volunteer must fill out this application
